File a Claim

Which product would you like to file a claim for?

To file a claim, please have these documents ready:

Certified, colour copy of the Death Certificate*
Return of Original Policy Documents or if unable to locate the beneficiary needs to complete the “Declaration of Lost Policy Form” .
Proof of Death Physicians Statement
Certified, Colour Copy of the Birth Certificate or Passport of the deceased.
Certified, Colour Copy of the Marriage Certificate or Divorce Decree (if the deceased name is different than listed on file at time of death).
Proof of Executor of Estate/Letters of Administration
Claimant Statement
Personal Verification Form – Beneficiaries.
Certified, Colour Copy of Valid Photo ID (government issued – passport or driver’s license)
Certified, Colour Copy of Marriage Certificate or Divorce Decree Certified (if the beneficiary’s name is different than listed on file at time of claim).
Certified, Colour Copy of Valid Proof of Address (utility bill or bank/credit card statement, cannot be a mobile/cell phone bill)
FATCA (Foreign Accounts Tax Compliance Act)/CRS (Common Reporting Standards) Form
